Interview/Intake & Quality Review Sheet

The information you obtain from the taxpayer's completed Form 13614-C, Intake/Interview & Quality Review Sheet will help you determine the payments and credits to report on the taxpayer's return. The income tax withholding, EIC, estimated payments, and additional child tax credit are the largest payments that volunteers will encounter.

Be sure to also review the intake and interview sheet:

• Page 2 – Income, questions that identify sources of income for which a taxpayer may be required to pay taxes
• Page 3 – Life Events. For the question “Did you pay estimated taxes to the IRS?”, if the answer is “yes,” ask to see the taxpayer’s record of payments, such as Form 1040-ES, Estimated Tax for Individuals.
